For patients with learning disabilities, it can be hard to understand the signs your body sends to tells you your health can be at risk. Please see the information posters below to help patients recognise these symptoms.

If you care for an adult with a learning disability it can be really helpful to have a plan in place in case your circumstances suddenly change.
In Hertfordshire social care follows a connected lives principle, which means they aim to support you to be able to live a full, happy and safe life with the connections in your community.

If you need extra help, or may do in the future, it is best to plan for this so that there isn’t a sudden crises. Asking for an assessment means people will help YOU to have the outcomes that work for YOU. Here is a link to Adult Care and Support www.hertfordshire.gov.uk/careandsupport
