New Patients

South Street Surgery welcomes new patients. If you would like to register with the Surgery please first of all check that you live within the practice area. Then complete an online form.

Alternatively, you can download the registration form below and bring it into the practice.

PRF1 Registration Form

Please make sure you complete your new registration form fully otherwise it may delay your registration with the practice. It is important to complete your town and country of birth along with any previous address and GP details. Primary Care Support England are responsible for authorising your registration with the practice and allocation of NHS numbers should one be required.

Change of Personal Details

If you have any changes you wish to make to your personal details or address, please complete our online Change of Details form.

Overseas Registration

If you have moved to live in England from overseas you can register for free health care services at the practice. Please ensure you complete the registration form and the supplementary questions as NHS care outside of the GP practice may not be free. You may be asked for proof of entitlement to receive free NHS treatment. Further information is available at www.nhs.uk/visitingengland
