Carers for people with a learning disability

If you care for an adult with a learning disability it can be really helpful to have a plan in place in case your circumstances suddenly change.

In Hertfordshire social care follows a connected lives principle, which means they aim to support you to be able to live a full, happy and safe life with the connections in your community.

Here is a short Video about connected lives – www.hertfordshire.gov.uk/yourconnectedlife

If you need extra help, or may do in the future, it is best to plan for this so that there isn’t a sudden crises. Asking for an assessment means people will help YOU to have the outcomes that work for YOU. Here is a link to Adult Care and Support www.hertfordshire.gov.uk/careandsupport

And here is information about having an Assessment www.hertfordshire.gov.uk/myassessment
