Health Care Assistant and Nurse Appointments

 

Nurse and HCA appointments 
The nursing team offer a range of services such as cervical smears, long term condition reviews ( asthma, diabetes, COPD, CHD, epilepsy) reviews, NHS health checks, wound care and vaccinations.  You may be sent a link to book directly online into one of these clinics for your review.  These appointments are not available to book directly without a link please send an e-consult or call us if you think you require one of these appointments.
Nurse-Led Minor Illness Clinic
Many conditions such as colds, sore throats, cough, flu, minor eye or ear problems, hay fever, minor skin problems & rashes can be treated at home. The NHS provides advice on a range of conditions Conditions A to Z – NHS
If your condition is ongoing or you are worried you please contact us via e-consult Consult Online from Home – South Street Surgery (webgp.com)  or call the practice ( 01279 465700).   You may be booked to see a Minor illness nurse our Physician Assistant or into our PCN same Day Access clinic.
PCN Same Day Clinic
Appointments are offered Monday – Friday in the afternoon in our PCN same day clinic which is available to all practice in Stort Valley and Villages PCN.  These appointments are for minor illness conditions which require a same day assessment such as earaches and sore throats.  You may be triaged to an appointment in one of these clinics.
